Apr 2013: We have moved to our new location!
After 13 years at Dunwoody Park, we outgrew our space and started looking for a new home for Arbor. We are now located at 4360 Chamblee-Dunwoody Road, Suite 370. This building offers a much nicer and more comfortable environment for our patients, as well as gives us the opportunity to customize our office to provide the best care.
Mar 2013: We are now using a new Practice Management Company and have moved to Electronic Medical Records.
Though this has been a tough transition, we are hopeful that the ease and convenience of computer records will improve our care and your experience. This system has great features, such as the Patient Portal, which allows you to access your lab results, request appointments, and pay bills online. Unfortunately, the process of transferring your health information to the new records can be time-consuming. This is especially true for patients who have been with us for a long time or who have extensive health histories.
We ask that you assist us with this process by clicking on the Patient Portal icon at the top of this page. Your PIN number to log on is 12345. You may change your PIN number when you log on. Please complete your health history before your next appointment. If unable to complete the health form before your next appointment, then please arrive at least 30 minutes prior to your appointment so that you may complete the information in our office.
